The business provides flexible, person-centred support to individuals with varying needs and works with clients, families, carers and relevant service providers to facilitate appropriate support and access to community resources.
Smart Disability Care is seeking a Community Worker to assess client needs, develop and implement support programs, monitor client progress, facilitate referrals and liaise with families, community organisations and other service providers.
ABOUT THE ROLE
The Community Worker will provide community-based support to individuals with disabilities, focusing on client assessment, support planning, service coordination and community participation.
The role involves interviewing clients, developing and implementing educational, training and support programs, monitoring outcomes, facilitating referrals, identifying appropriate community resources, and liaising with families, carers and relevant community, welfare and government organisations.
The position requires strong communication, organisational and stakeholder engagement skills, together with a sound understanding of person-centred disability and community support.
KEY RESPONSIBILITIES
Client Assessment and Support Programs
- Assess clients' individual needs, circumstances, goals and support requirements and contribute to the development of appropriate support plans.
- Interview clients and, where appropriate, their families or carers to assess the nature and extent of difficulties affecting their wellbeing, independence and community participation.
- Plan, develop and implement educational, training and support programs tailored to clients' individual needs and identified goals.
- Provide information and practical support to assist clients and families to understand available services, resources and support options.
- Review client circumstances and support requirements and recommend appropriate changes to support strategies where required.
Client Progress and Referrals
- Monitor and evaluate the progress of clients against identified support goals and objectives.
- Maintain accurate client records and prepare progress reports and other documentation relating to client outcomes and service delivery.
- Identify circumstances where clients require additional assistance and refer them to appropriate health, welfare, disability and community agencies.
- Liaise with relevant service providers to facilitate coordinated and appropriate support for clients.
- Follow up referrals and service arrangements to assist clients in accessing appropriate support and community resources.
Community Resources and Service Coordination
- Assess client and community needs and identify available resources relating to health, welfare, housing, employment, training and other community services relevant to clients.
- Research and maintain knowledge of local community resources and services available to people with disabilities and their families.
- Assist clients to access appropriate community, welfare, health and support services consistent with their individual circumstances and goals.
- Promote awareness among clients and families of relevant community resources, support services and opportunities for community participation.
- Identify gaps or emerging needs in service provision and communicate relevant information to management and appropriate stakeholders.
Community and Stakeholder Liaison
- Liaise with community groups, welfare agencies, government bodies and private organisations regarding client support needs and community service matters.
- Establish and maintain professional relationships with external organisations and service providers to facilitate appropriate client referrals and support.
- Work collaboratively with families, carers and other professionals to promote coordinated, person-centred service delivery.
- Promote inclusion, independence and participation of people with disabilities within the broader community.
- Participate in community engagement activities and initiatives that increase awareness of available disability and community resources.
Family, Disability Support and Reporting
- Support families and carers by providing information, education and practical guidance relevant to the needs of people with disabilities.
- Assist clients to develop greater independence, confidence and participation in daily and community activities in accordance with their individual support objectives.
- Identify social, emotional, practical and community barriers affecting clients and assist in developing appropriate support strategies.
- Prepare reports, case documentation and other written material for management, government bodies, agencies and relevant service providers as required.
- Contribute to service improvement initiatives and provide feedback regarding client needs, community resources and opportunities to improve support outcomes.
MANDATORY REQUIREMENTS
- Diploma or higher qualification in Community Services, Social Work, Disability Services, Welfare or a related discipline.
- At least 2 years relevant professional experience in community services, disability support or a related field.
- Experience in client assessment, support planning and service coordination.
- Knowledge of disability, health, welfare and community support services.
- Strong communication, documentation and stakeholder liaison skills.
